The Amyloidosis Foundation (AF) is committed to increasing medical community awareness and improving the lives of those affected by these diseases. This is an ambitious task and we need your help!
The Amyloidosis Foundation provides assistance to patients through our web site, sponsors attendance at medical and scientific conferences around the country, produces educational materials, and have over 20 support groups nationwide. Welcome to the AF's web site. Here we have outlined some fundraising and gifting programs that allow you to directly help those affected by amyloidosis and assist in creating awareness of this disease.
Deciding on what to do can be confusing. The AF can help you. We are ready to provide you with the assistance you'll need to make your fundraiser a success.
FIRST STEP - Determining Your Fundraising or Gifting Objective
- How much money do you want to raise? We suggest a minimum of $1000
- When are you planning on starting your fundraiser?
- What is your deadline for raising the funds?
- How many participants do you think will be involved in the fundraiser?
- Do you have a volunteer base to help organize and run the fundraiser?
- How important is the ease of administration?
- How would you measure the success of your fundraiser?
- Will you need to meet licensing & insurance requirements? Please check with your State/County/Local Municipality regarding this for your event.
- Can you get business sponsors? Many local businesses, large and small, can help by underwriting national or local events, patient services programs and/or educational materials.
- In what form will donation be accepted? Contributions to the AF are tax deductible. Checks can be made payable to the Amyloidosis Foundation or donations can be accepted via our secure credit card service at http://www.amyloidosis.org/contributions.asp. If you get cash (currency), it’s probably best to send us a check for the amount received.

A greater number of people are now asking their friends and family to donate money to a charity rather than give flowers at funerals. Flowers fade with time but memorial donations can help those who continue to live with amyloidosis and their families and caregivers.
If you feel that you want the AF to benefit in this way, then tell your family or the executors of your will that you would prefer donations to be made to the Amyloidosis Foundation instead of flowers.

Contact us and we will supply (by express mail if needed) an agreed upon quantity of lapel ribbons, donation cards, pamphlets free of charge.

Planned Giving…
Include a Gift in Your Will
Wills and Bequests - Many people have asked about language that could be included in a will or bequest to donate all or part of an estate to the Amyloidosis Foundation. The simple format below has been suggested, however, we recommend that you review with your attorney what your intentions are and whether more specific language might be necessary to convey those intentions:
Simple format
"I, [name], of [city, state, ZIP], give, devise and bequeath to the Amyloidosis Foundation, Inc. (AF), with national offices located at 7151 N. Main St. Suite 2, Clarkston, MI 48346 [insert written amount of gift, percentage of the estate or residuary of estate, or description of property] for its unrestricted use and purpose."
To donate a specific sum:
"I give the sum of ________ dollars ($_____) to the Amyloidosis Foundation Inc., a non-profit corporation presently located at 7151 N. Main St. Suite 2, Clarkston, MI 48346, to be used for its general charitable purposes."
To donate a percentage of an estate:
"I give ____% of the residue of my estate to Amyloidosis Foundation, a non-profit corporation presently located at 7151 N. Main St. Suite 2, Clarkston, MI 48346, to be used for its general charitable purposes."
To make a restricted bequest to the Awareness Fund or Support Fund (can also be done as a percentage):
"I give the sum of _______ dollars ($_____) to Amyloidosis Foundation, a non-profit corporation presently located at 7151 N. Main St. Suite 2, Clarkston, MI 48346, to be held as a separate fund, designated as the _______________ Fund. The entire net income of the fund and such part of the principal as the Board of Directors shall determine in the absolute discretion of said Board shall be used for ____________________."
